Fees & Insurance
I accept self-pay and am also in-network with most Blue Cross Blue Shield plans and will file claims on your behalf with your permission. I require payment or co-pay in full at the time of service. I understand the importance of finances when deciding to seek services and should you opt for self-pay I am also able to provide you with a detailed receipt (or super bill) containing all necessary information for you to submit to your insurance carrier for potential reimbursement with your Out-of-Network benefits. If you are interested in utilizing insurance, I recommend you contact your insurance carrier directly to inquire about your specific outpatient behavioral health benefits. My fee schedule can be found below. Please feel free to contact me about fees directly if you have any specific questions.
Fee Schedule:
Below is my basic fee schedule. Please note these rates include time spent engaging in comprehensive treatment planning and review of assessments in between sessions, coordinating any care with clients' interdisciplinary treatment teams, and any online payment processing fees. This fee schedule is not inclusive and other services may be billed as requested by my clients.

Initial Intake Assessment (60-90 minutes) - $250

Questions to ask your insurance about "out of network" benefits:

Do I have mental or behavioral health benefits?

Payment:
Payment is due at the time of the service. In the event you need to cancel or reschedule an appointment, a 48-hour notice is requested, or a cancellation fee will incur. A valid form of payment on file is required for all clients prior to initiating services. I accept debit, credit, HSA, and FSA cards. Your card will be charged through the client portal.
Cancellation Policy:
I have a 48-hour cancellation policy. This means that you will be charged a late fee of $150, which is near the full session rate, if you cancel less than 48 hours in advance of the session. This also applies to no show appointments.
Good Faith Estimate:
You may obtain a good faith estimate of my charges upon request prior to scheduling with me.
The No Surprises Act is a federal law which provides you with the right to a good faith estimate of the cost of services at my practice. However, state licensing board rules require me to provide you with the actual cost of my charges in a written informed consent form to which you must agree prior to my providing services. That will be available to you prior to you being seen for services and prior to any billing. In most cases it is impossible to estimate how many sessions you will need, and that will not be determined until your concerns are evaluated and will also vary based on the progress that you make, which depends in part on your efforts with the process. You will be free to discontinue services at any time or the services may otherwise be terminated in accordance with the informed consent form language.

Although the No Surprises Law says that you may initiate a dispute process if the actual charges are substantially in excess of the Good Faith Estimated charges, i.e. if you are charged $400 more than the estimated cost for a session or for the total estimate provided, that is unlikely to happen and would be a violation of licensing board rules, since you will be agreeing up front to actual charges per session prior to being seen. Dispute information is available upon request. Any changes to my fees will require a change in the informed consent form fees, which you must agree to prior to having them go into effect.
